Abstract

INTRODUCTION AND OBJECTIVES: Salvage focal cryotherapy of the prostate involves treating the recurrent cancerous lesion in the prostate, avoiding normal surrounding structures, thus minimizing the side effects of therapy. The purpose of our study was to compare the outcomes of salvage focal and whole gland cryotherapy in patients who were hormone naive. METHOD(S): Utilizing the Cryotherapy On-Line Database (COLD) registry, we identified hormone naive patients who underwent salvage focal or whole gland cryotherapy of the prostate for recurrent prostate cancer (CaP) between 2000-2013 after prior failed treatment with primary radiotherapy (XRT). Statistical analyses (Kaplan-Meier) were performed comparing age, Gleason sum, PSA and stage prior to cryotherapy, time to biochemical recurrence (BCR) using Phoenix criteria (PSA nadir 6 months after cryotherapy + 2 ng/ml), potency, urinary incontinence, retention, and presence of rectourethral fistula. RESULT(S): We identified a total of 137 hormone naive patients with recurrent CaP following primary XRT; of which 26 and 111 underwent salvage focal and whole gland cryotherapy, respectively. There was no significant difference between groups in median age [71 years (IQR 67-76), p=0.83)], sum of Gleason score [Gleason 6 (IQR 6-7), p=0.43)], PSA [focal 4.5 ng/ml (IQR 3-5.7), whole gland 4.7 ng/ml (IQR 2.8-6), p=0.43] and stage (
